Medical Needs
Parents/carers have the prime responsibility for their child’s health and should provide school with information about their child’s medical condition on the admissions form.
If your child’s needs change, please remember to inform the school in writing immediately, so that our records can be amended accordingly.
We are unable to administer any prescribed medication to children unless we have a signed consent form.
Please do not send your child to school with medication to self-administer, even if they are over the age of 16.
To enable us to be able to administer medication to your child, we ask that you complete and sign a Crowdys Hill School occasional medication form, requesting that medication is administered to your child at school.
The form should be handed into reception, together with the medication.
All medication should be in the original container, clearly labelled with your child’s name and the dosage and administration instructions.
Crowdys Hill School will not accept medicines that have been taken out of the container or make changes to dosage on parental instructions.
For safety reasons, all medication, with the exception of asthma inhalers, must be held in the medical room. Please make sure you have informed us if the medication needs to go home daily.
